New Patient Information:
Please arrive at your new patient appointment with completed new patient forms, insurance cards, driver’s license or ID card, and a complete list of your current medications. You can expect your first appointment to last approximately one hour. We may ask for you to obtain a chest x-ray prior to your appointment with us. To obtain an x-ray, please check in at the Radiology Department on the 2nd floor of the main tower on the Northwest Hospital campus about one hour prior to your appointment with us.
Forms:
Please download and complete these forms and bring them with you to your consultation. Completing them before your appointment will reduce your wait time when you arrive at our office.
Prescription Refills:
Most prescriptions are written for 30 days with additional refills for a year. Some exceptions may apply. If you are in need of refills, please contact your pharmacy directly with your prescription number. The pharmacy will contact us to confirm your prescription. Some mail order prescriptions require a written order and we are happy to provide one as needed. Narcotics prescriptions must be written by a physician and cannot be called in over the phone or faxed. All written prescriptions for narcotics need to be picked up in person in our office. We are unable to call in prescription refills after hours or on weekends.
Phone Messages:
Because our physicians provide critical and emergency care to many hospitalized patients, we may be called away from our offices periodically. If we are unable to answer your phone call in person, we offer a voice messaging service for your convenience. Please leave a detailed message and we will return your call before leaving the office in the evening.
Hospitalization:
Northwest Respiratory patients in need of hospitalization will be admitted to Northwest Hospital & Medical Center unless requested otherwise.
